Alabama Medicaid Provider Enrollment and Credentialing: The Gainwell Portal, ACHN Agreements, Revalidation and Palmetto GBA
Alabama Medicaid provider enrollment is an online application in the Alabama Medicaid Agency's Provider Enrollment Web Portal, processed by its fiscal agent Gainwell Technologies, which the Agency's manual says approves, returns or denies each application within ten business days of receipt. Primary care groups that want ACHN rates sign two more agreements, Medicare enrollment is filed in PECOS for Palmetto GBA to process, and patients covered through the Railroad Retirement Board require a separate Railroad PTAN.
Updated October 23, 2026. Every standard here is tied to the Alabama Medicaid manual chapter, Agency alert, form or federal page behind it, dated to our reading. Gaps in what Alabama publishes are named as gaps. Treat this as operating guidance rather than legal advice, and check with the Agency before billing on it.
We enroll providers with Alabama Medicaid, the ACHN program, Medicare and the commercial plans. To have us take the whole file, start credentialing now and get a secure start link within one business hour.
Alabama Credentialing at a Glance
Alabama is simpler than most Medicaid states because it has no risk-bearing Medicaid MCOs to contract with one by one, but the separate files still each have their own owner and clock. The standards below are the ones the Agency, CMS or Palmetto publish; they describe the process, not any particular application.
| File | Owner | Where it is filed | Published standard | Source |
|---|---|---|---|---|
| Alabama Medicaid enrollment | Alabama Medicaid Agency, through Gainwell | Provider Enrollment Web Portal plus barcoded documents | Approved, returned or denied within 10 business days of receipt | Provider Manual ch. 2 |
| ACHN primary care participation | Medicaid Managed Care Operations and the regional ACHN entity | PCP Group Enrollment Agreement uploaded in the Interactive Web Portal | Agreement term October 2024 to September 2029 | ACHN for Providers |
| Revalidation | Alabama Medicaid Agency | Revalidation Facsimile via the secure portal | By the date on your notice; 2026 off-cycle round under way | ALERT, June 30, 2026 |
| Medicare Part B | Palmetto GBA (Jurisdiction J) | PECOS | Online with no site visit or development: 95% within 15 calendar days, all within 50 | CMS PIM ch. 10 |
| Railroad Medicare | Palmetto GBA, Railroad Medicare unit | PTAN Lookup and Request Tool | Allow up to 30 days | Palmetto GBA |
Alabama Medicaid enrolls only physicians who are fully licensed and hold a current license to practice, per chapter 2 of the Provider Manual; residents not yet licensed by the Alabama Board of Medical Examiners bill under the supervising physician's NPI.
Alabama Medicaid Provider Enrollment in the Gainwell Portal
The Alabama Medicaid Provider Enrollment Web Portal is linked from the Agency's Provider Enrollment and Forms page. Its user manual (version 21.0, modified October 22, 2025) explains a few things that trip people up before they reach the first screen:
- No login is needed to start. But if you choose "finish later," you set a password tied to a tracking number and tax ID, and the manual is blunt: "passwords cannot be reset." Write it down where the next person can find it.
- The enrollment type drives everything. Picking individual, group or facility decides which screens and documents follow, so choose carefully.
- The contact email is where problems go. Gainwell uses it for questions and notices on the application.
- The requested effective date is a request. The manual says the provider "is NOT guaranteed this effective date."
The workflow, from chapter 2 of the Provider Manual:
- Complete the online application. Alabama's Participation Requirements document lists the qualifications and documents for your provider type.
- Submit, then send the supporting documents with the barcoded cover sheet the portal produces. Forms that need a wet signature (the EPSDT agreement, Plan First agreement, telemedicine agreement and the primary care rate attestation) cannot go electronically; EFT forms need a wet signature but may be faxed.
- Gainwell decides within five business days whether corrections are needed and approves, returns or denies within ten business days. Returned applications are logged out and back in, so the clock runs from your resubmission.
- On approval, Gainwell mails an enrollment notification letter listing the NPI within two business days. A denial letter gives the reason and a Medicaid contact for appeal; an incomplete file gets an email listing what is missing.
Enrollment is per location and per person: a provider with several locations enrolls each one under the same NPI, and every physician in a group enrolls under their own NPI, per the Agency's provider enrollment FAQ. EFT and electronic remittance advice are both required for payment under the manual.
Documents Alabama Medicaid Requires
Alabama spells out its documents by provider type in the Participation Requirements (revised December 2025, effective January 1, 2026). The baseline:
| Who | Documents |
|---|---|
| Every application | W-9 tax form (it may be faxed with the barcoded cover sheet), plus where applicable: CLIA certificate, DEA certificate, EPSDT agreement, Plan First agreement, telemedicine service agreement, mammography system certification; dental-only IV sedation and mobile dental facility certifications |
| Group or facility | Provider Disclosure Form; voided check or bank letter for EFT; corporate board of directors resolution and articles of incorporation if incorporated; Certificate of Authority to do business in Alabama for out-of-state corporations; the application fee if the provider type owes one |
| Physician | License in Alabama or the state of service; DEA copy if applicable; a written explanation for any limited license, without which the fiscal agent will not enroll |
| Nurse practitioner (CRNP) | RN and CRNP licenses; national certification in the practice area; for independent CRNPs, the protocol signed by a collaborating physician; proof of prescriptive authority to prescribe |
| Most professionals | Civil Rights Compliance Information Request Package: the General Data and Statement of Compliance sections if Medicare-enrolled, the whole package if not |
A notable Alabama rule for chiropractors: the same document says they must be Medicare certified and are enrolled only for services to QMB recipients or to recipients referred as a result of an EPSDT screening.
DEA registrations get their own handling. The Agency asks prescribers to keep a copy on file, uploaded through the Medicaid Interactive Web Portal or faxed to (334) 215-7416 with the portal's barcode cover sheet, and says Medicaid applies it to every service location matched by NPI and license number. Claims for controlled drugs from a prescriber without a registered DEA number deny.
Why Alabama Medicaid Applications Are Denied
Alabama publishes its own list. The Agency's December 19, 2023 alert on provider enrollment names the most common denial reasons:
- Name mismatch. The name on the application has to match every document. The Agency's example: apply as "Benjamin J. Smith" with a license reading "James Smith" and the application denies.
- Missing documents or forms, including the Civil Rights forms required on every application.
- An incorrect Trading Partner ID. Group and facility applications need a valid one, requested on the Provider Trading Partner ID Request Form; Gainwell mails a PIN letter with the ID.
- Invalid signatures. Each individual signs their own part of a group application, and the group application is signed by an authorized representative, such as a managing employee listed on the ownership disclosure.
- Collaboration rules. An independent CRNP's collaborating physician does not have to be enrolled with Alabama Medicaid, but a physician-employed nurse practitioner's collaborator must be actively enrolled in the same group, and every NP and PA needs an active collaborating physician on their license.
After approval, changes go through the secure portal's Forms menu; the Agency stopped accepting faxed enrollment updates on January 2, 2017, according to its Provider Enrollment Forms page.
How to Check Alabama Medicaid Enrollment Status
To check Alabama Medicaid enrollment status, open Enrollment Status in the Provider Enrollment Web Portal and enter the tracking number and tax ID from your submission; the user manual says both are required. By phone, Gainwell Provider Enrollment is (888) 223-3630, Monday to Friday, 8 a.m. to 5 p.m. Central, and chapter 2 says the representative will ask for the provider's name, NPI, telephone number, and SSN or FEIN.
Once approved, the Agency's forms page links instructions for downloading the group and individual welcome letters from the web portal. The welcome letter and remittance advice carry the Alabama Medicaid group billing ID, which is a different number from the NPI and is what the ACHN agreement asks for. For claim and payment questions after enrollment, the Gainwell Provider Assistance Center is 1-800-688-7989, per the FAQ.
Medicare First or Medicaid First? Alabama's Answer and the Crossover Letter
Apply to both at once. Chapter 2 says physicians and other individual practitioners "should not wait until they have obtained Medicare approval" to complete the Medicaid application; Alabama assigns a Medicaid effective date that may change to the Medicare effective date once Medicare finishes.
The follow-up matters as much as the application. For Medicare claims to cross over to Medicaid automatically, the manual asks providers to fax or mail a copy of the Medicare notification letter, with the NPI and the secondary identifiers for every service location, to Gainwell Provider Enrollment. Its fax is (334) 215-4298. Until that letter is on file, dual-eligible claims sit unpaid on the Medicaid side even though both enrollments exist.
ACHN: Alabama Coordinated Health Network Agreements for Primary Care
Alabama Medicaid does not run on capitated health plans. The Agency's ACHN overview describes seven predefined regions with one network in each; the networks run care management, do not take full financial risk, and do not pay providers, because Medicaid pays providers directly. Dual eligibles are excluded from the ACHN population.
A primary care group that wants the ACHN participation rates and bonus payments signs two agreements beyond its Medicaid enrollment: the PCP Group Enrollment Agreement with Medicaid and a participation agreement with the ACHN entity in its region. The Agency's application checklist (updated May 2024) flags the details that send these back:
- Use the group's current Alabama Medicaid group billing ID, name, tax ID and physical address exactly as enrolled.
- List only physicians and collaborating NPs or PAs who are already enrolled and active under the same tax ID and NPI; "pending enrollments should NOT be listed."
- Give their Medicaid provider IDs, not NPIs.
- Complete the admitting privileges section, or have the admitting physician sign Attachment B.
- If the agreement follows a new enrollment that created a new group billing ID, email Managed Care Operations at ACHN@medicaid.alabama.gov, or attribution can be missed.
The upload steps (April 14, 2026): log into the Interactive Web Portal secure site, open Trade Files, then Forms, choose "ACHN PCP Group Agreement Documentation," attach the agreement and submit. Save the barcode cover sheet it generates; it is how you add documents later or fax a paper copy to (334) 215-7416.
| Region | ACHN entity | Provider line |
|---|---|---|
| Central | My Care Alabama Central | 1-855-288-8361 |
| East | My Care Alabama East | 1-855-288-8366 |
| Jefferson/Shelby | Alabama Care Network Mid-State | 1-833-296-5245 |
| Northeast | North Alabama Community Care | 1-855-640-8827 |
| Northwest | My Care Alabama Northwest | 1-855-500-9470 |
| Southeast | Alabama Care Network Southeast | 1-833-296-5246 |
| Southwest | Gulf Coast Total Care | 1-833-296-5247 |
Provider lines are from the Agency's ACHN regional contacts sheet (April 16, 2026), which also lists the counties in each region. Separately, Alabama's primary care "bump" enhanced rate is claimed with its own self-attestation form (revised April 2, 2026) on the forms page.
Screening, Site Visits, Fingerprints and the Application Fee
Alabama screens by federal risk level, and its 2026 notices show where each provider type sits. The June 5, 2026 alert lists the high-risk group as DME and medical supply dealers, home health agencies, hospice, nursing facilities, ABA therapy and behavioral health. Physicians, chiropractors, dentists, psychologists, podiatrists, therapists, CRNPs, PAs, hospitals, labs, pharmacies and OPR-only enrollments are on the moderate-or-limited list.
For fingerprints, chapter 2 applies 42 CFR 455.450: high-risk providers, providers moved to high risk by a payment suspension or an existing overpayment, providers excluded in the past 10 years, and those applying within six months after a moratorium lifts, plus anyone with 5% or more ownership in them. Alabama notifies each person by letter, and they have 30 days to be fingerprinted at the Alabama Law Enforcement Agency or their nearest law enforcement agency. If Medicare already ran the check, Alabama Medicaid may rely on it. The Agency's enrollment page adds that skilled nursing facilities now get a site visit at initial enrollment and at revalidation every five years.
The application fee is $750 for calendar year 2026, paid by certified or cashier's check to the Alabama Medicaid Agency with the initial application or revalidation. Individual physicians and non-physician practitioners do not pay. The Agency's fee list covers ASCs, CMHCs, DMEPOS, dialysis, FQHCs, home health, hospice, hospitals, IDTFs, labs, nursing facilities, private duty nursing, radiology, rural health clinics, transportation, and therapy groups (physical, speech, occupational and ABA). Institutions that paid Medicare or another state, or are enrolled in Medicare, another state's Medicaid or CHIP, are exempt if they document it; hardship requests go by letter to the fiscal agent, and fee questions to the Enrollment and Sanctions Unit at (334) 242-5000.
Alabama Medicaid Revalidation and the 2026 Off-Cycle Round
Alabama tells you when to revalidate rather than publishing a fixed physician cycle. Chapter 2 says providers are notified when they are scheduled, and failure to revalidate with the right documents end-dates the file; once closed, the provider must submit a new enrollment application. The Agency posts a monthly list of providers due, most recently the October 2026 list dated October 20, 2026, on its enrollment page.
In 2026 Alabama added an off-cycle round at CMS's request. Its June 30, 2026 alert to all providers describes a swift revalidation for high-risk providers and a 24-month revalidation for moderate and limited-risk providers, with notices sent in phases. When yours arrives:
- Download the Revalidation Facsimile from the Medicaid secure web portal.
- Complete it with the forms that apply: the Provider Agreement; the Individual Disclosure Form (individuals) or Facility/Group Disclosure Form (entities); and, only if something changed, a W-9, CLIA or DEA certificate; plus EPSDT, Plan First, mammography and corporate board resolution forms where relevant.
- Submit it all through the secure portal by the date on the notice.
The same list appears in the Agency's revalidation documentation requirements. Chapter 2 separately lists the revalidation facsimile, provider agreement, board resolution, EPSDT agreement and ACHN enrollment among forms that cannot be sent electronically, so read your notice for the submission method it specifies.
Inactivity closes files too. The manual says a provider with no claims for a consecutive 18 months is disenrolled and must re-enroll.
Ordering, Prescribing and Referring Providers in Alabama
Every practitioner who orders, prescribes or refers for an Alabama Medicaid recipient has to be enrolled, even one who never bills. The Agency's FAQ puts it plainly: "Claims will deny if provider is not enrolled." OPR applications use the same Provider Enrollment Application Portal. Chapter 2 adds that licensed physicians in a residency program may enroll so their prescriptions to Medicaid recipients can be filled, while services performed as part of the residency are billed by the supervising physician.
Behavioral health clinicians have had to enroll individually since April 1, 2019: the Agency's September 20, 2018 alert required LPCs, ALCs, LMFTs, LMSWs and LICSWs to enroll with their own NPIs, one application per location, and is also where the "up to 40 days" enrollment estimate comes from.
Medicare in Alabama: Palmetto GBA, Jurisdiction J
Alabama's Medicare Part A and B contractor is Palmetto GBA, Jurisdiction J, which also serves Georgia and Tennessee. The CMS provider enrollment contact list gives Palmetto's enrollment number as 877-567-7271 and the Part B enrollment address as P.O. Box 100306, Columbia, SC 29202. File the CMS-855I for an individual (reassignment to a group is part of that form) or the CMS-855B for a group in PECOS. Our Medicare provider enrollment page covers which form applies to whom.
Palmetto follows the CMS timeliness table in Program Integrity Manual chapter 10 rather than publishing its own:
| Application | 95% within | All within |
|---|---|---|
| Web (PECOS), no site visit, development or fingerprinting | 15 calendar days | 50 calendar days |
| Web (PECOS), with any of those | 50 calendar days | 85 calendar days |
| Paper, with none of those | 30 calendar days | 65 calendar days |
| Paper, with any of those | 65 calendar days | 100 calendar days |
Retroactive Medicare billing is capped by 42 CFR 424.521 at 30 days before the effective date for physicians and non-physician practitioners whose enrollment was delayed by circumstances outside their control, or 90 days after a Presidentially declared disaster.
Railroad Medicare in Alabama
Patients whose Medicare comes through the Railroad Retirement Board are billed to Palmetto's Railroad Medicare unit under a different PTAN from the one Jurisdiction J issued. Palmetto's Enroll in Railroad Medicare page (published January 13, 2026) lays out the order: finish Part B enrollment, request the Railroad PTAN through the PTAN Lookup and Request Tool without filing another CMS-855, allow up to 30 days, and then enroll separately for electronic claims. The Railroad PTAN takes the same effective date as the Part B PTAN.
Request it the week the Part B approval arrives. Without it, the first Railroad Medicare patient's claim rejects, and by then nobody remembers that a second PTAN was ever needed.
Commercial Plans, Blue Cross Blue Shield of Alabama and CAQH
Alabama's commercial credentialing runs largely on CAQH. CAQH describes Blue Cross and Blue Shield of Alabama, the state's largest health plan, as using the CAQH Credentialing Suite to obtain provider data, verify primary sources and monitor sanctions (CAQH case study, May 2023). ProView profiles expire from the plans' point of view unless re-attested on CAQH's 120-day schedule (CAQH quick reference), so a single lapse can idle BCBS of Alabama and every other carrier at once. For the profile itself see CAQH explained; for UnitedHealthcare, Aetna, Cigna, Humana and the rest, use our credentialing hub by payer.
We did not find an Alabama statute or regulation that sets a deadline for commercial insurers to finish credentialing, so we quote none; ask each plan for its own published turnaround and log the answer.
Why Alabama Credentialing Files Stall
When an Alabama file sits, the cause is usually provider data that does not line up, and the Agency's own denial list reads like a catalogue of it:
- Names that vary across documents: a middle name used on the application and not on the license is enough to deny.
- The barcoded cover sheet never used, so documents arrive unmatched to the application.
- A lost portal password, which cannot be reset, stranding a half-finished application.
- No Trading Partner ID on a group application.
- Collaborating physician records that do not match for NPs and PAs, which also blocks the ACHN agreement.
- Pending providers listed on an ACHN agreement, which the checklist forbids.
- The Medicare letter never sent, so dual-eligible claims do not cross over.
- Revalidation notices missed, ending in an end-dated file and a new application.
- No Railroad PTAN when the first railroad retiree arrives.
So we check before we submit and follow up after: one file compared field by field across the application, NPPES, the license record, CAQH and PECOS, then weekly follow-up with every payer, recording who we spoke to, when, and the reference number they gave.
What to Gather Before You Start
For an Alabama physician or practitioner: individual and group NPIs, the W-9 and IRS letter with the exact legal name, the license and its dates, DEA registration, CLIA if you run a lab, board certification, malpractice certificate, collaborating physician details for NPs and PAs, the group's ownership and managing-employee information for the disclosure form, a voided check or bank letter, a Trading Partner ID for the group, an attested CAQH ProView profile, and Palmetto's approval letter when Medicare is done. The checklist we work from, and the secure way to send it, is on our intake page.
For how credentialing, enrollment and contracting differ, see what provider credentialing is; the national Medicaid framework that Alabama's process sits inside is on Medicaid provider enrollment. Other state guides: Georgia, Florida and Kentucky.
Frequently Asked Questions
How do I enroll as an Alabama Medicaid provider?
Complete the online application in the Alabama Medicaid Provider Enrollment Web Portal, which Gainwell Technologies runs for the Alabama Medicaid Agency, then send the supporting documents with the barcoded cover sheet the portal gives you after you submit. Gainwell reviews the file against the Agency's Participation Requirements for your provider type and approves, returns or denies it. Each location, and each physician in a group, is enrolled under the provider's own NPI.
How long does Alabama Medicaid provider enrollment take?
The April 2026 Alabama Medicaid Provider Manual says Gainwell approves, returns or denies each application within ten business days of receipt, decides whether corrections are needed within five business days, and mails the approval letter within two business days of approval. A returned application restarts that cycle when you send it back, and an older 2018 Agency alert told providers enrollment "can take up to 40 days," so start well before the first Medicaid visit.
What documents does Alabama Medicaid require?
Every application needs a W-9, plus, where they apply, a CLIA certificate, DEA certificate, EPSDT agreement, Plan First agreement and telemedicine agreement. Group and facility enrollments add the Provider Disclosure Form, a voided check or bank letter for EFT, a corporate board resolution and articles of incorporation for incorporated businesses, and a Certificate of Authority for corporations formed outside Alabama. Most providers also submit sections of the Civil Rights Compliance package.
How do I check my Alabama Medicaid application status?
In the Provider Enrollment Web Portal, open Enrollment Status and enter your tracking number and tax ID. By phone, call Gainwell Provider Enrollment at (888) 223-3630, Monday through Friday, 8 a.m. to 5 p.m. Central; the representative will ask for the provider's name, NPI, phone number, and SSN or FEIN.
Do I need to enroll with an ACHN in Alabama?
Only if you are a primary care physician group that wants the ACHN participation rates and bonus payments. Alabama Medicaid is not run by risk-bearing MCOs; it uses seven regional Alabama Coordinated Health Networks for care coordination, and Medicaid still pays providers directly. To participate, a PCP group signs two agreements on top of its Medicaid enrollment: the PCP Group Enrollment Agreement with Medicaid and a participation agreement with the ACHN entity for its region.
Should I wait for Medicare approval before applying to Alabama Medicaid?
No. The Agency's manual tells physicians and other individual practitioners not to wait. Alabama Medicaid assigns an effective date, which may change to the Medicare effective date once Medicare enrollment finishes. After Medicare approves you, fax or mail the Medicare notification letter to Gainwell Provider Enrollment so Medicare claims cross over automatically.
Does Alabama Medicaid charge an application fee?
Not to individual physicians or non-physician practitioners. Institutional providers pay $750 for calendar year 2026 by certified or cashier's check made out to the Alabama Medicaid Agency, unless they already paid Medicare or another state, or are enrolled in Medicare, another state's Medicaid or CHIP, and document it. The fee list includes therapy groups (physical, speech, occupational and ABA therapy), labs, home health, hospice, radiology and ambulatory surgical centers.
Why was my Alabama Medicaid application denied?
Alabama's December 19, 2023 alert lists the most common reasons: the provider's name on the application does not match the license or other documents; required attachments are missing; a group or facility application lacks a valid Trading Partner ID; signatures are invalid (each individual signs for themselves, an authorized representative signs for the group); and nurse practitioner or physician assistant collaborating-physician rules are not met.
Can an Alabama Medicaid provider number be closed for inactivity?
Yes. The Provider Manual says a provider who submits no claims for a consecutive 18-month period is disenrolled and must re-enroll to become active again. A file closed for missing a revalidation also requires a brand-new application.
Who is the Medicare contractor for Alabama?
Alabama is in Medicare Jurisdiction J, where Palmetto GBA is the A/B MAC (Georgia and Tennessee share it). Its enrollment line on the CMS contact list is 877-567-7271. Railroad Medicare is also handled by Palmetto, but through a separate PTAN requested after Part B approval.
Alabama Provider Credentialing Services
